While medical aesthetics such as Botox treatment and filling are added to the new non-surgical methods approximately for 20 years, they do not give results as surgical methods do. However, applying little details and necessary touch-ups make the patient feel and look better.
Dr. Ali Kerim Diler, having attended the international medical aesthetics congresses and followed all the developments in the field, states that the reason why medical surgery is preferred by more and more people in recent years is that people can make these applications in aesthetics without getting away from their social life. Dr. Ali Kerim Diler's suggestion to those who want to have surgery is that they should choose a doctor they can trust. Here is our talk with Dr.Ali Kerim.
What is medical aesthetics? What is its difference from aesthetic surgery? Can we call aesthetic aesthetics for medical aesthetics?
Biomedical technologies have improved especially in the last 20 years both in the field of biomedical devices as well as in the softening of soft tissue, wrinkles and lines filling materials. Non-surgical methods often do not give radical results as surgery does. But with little touch-ups, it makes the patient feel and look better. On the other hand, it has nothing to do with financial price as in surgical procedures. It is possible to look like three to five years younger with fillings made within the time of lunch. With the appropriate filling material, the desired results can be achieved without paying as much as in surgical operations. In short, we can define medical aesthetics as the one to provide non-operative solutions to aesthetic defects and providing psychophysical balance.
Is filling now more preferred instead of the Botox in ranging from curing wrinkles to the treatment of migraine? What is the difference between Botox treatment and filling?
Botox is actually used to cure the lines formed around the forehead and eye due to the contraction of dynamic muscles called mimic lines. It is attached to acetylcholine, which is a muscle and nerve-mediated intermediate, which also prevents nerve conduction from acting in muscle. This does not mean paralysis. We have a motor warning, a sensory stimulus. Botox treatment doesn't affect our feelings because acetylcholine is not the substance that makes us get feelings. Botox is actually a liquid substance that reduces or completely stops the muscle movements. Our tendency is to reduce muscle movements; not to stop them. Natural-looking Botox treatment is also possible. For example, according to theoretical knowledge, if an eye area is completely blocked by 12 units, a natural appearance can be obtained by using seven or eight units, not necessarily 12 units. A more appropriate result is obtained with the patient's age and social status. A woman who is 60 years old and a grandmother doesn't need to have overly tense skin. Even if the patient comes with such a request, I do not accept. We have now reached this point in communicating with patients.
So how do you evaluate the fillings made with one's own lipid?
I think filling with a person's own lipid is a bit rougher. Let's say the nose-lip groove or the cheekbone needs a filling of two cc. When you fill with a person's own fat tissue, this filling is gradually reduced. However, the filling with the substance can last longer depending on how much you inject.
How often do you need Botox Treatment or filling?
The patients usually feel that they need to have Botox or filling again, but it should be done every six months. In fact, this can be extended to 8-12 months. The frequency of use of the muscle can be reduced over time. When you do not re-Botox after a while, the muscle gets its old form again. It is necessary not to block the muscle completely and not to stop the functions of the muscle. Because small capillaries pass through the muscle and they feed the skin.
What are the new skin renewal applications?
Focused sound waves used in the technique of Ultherapy provide a tightening by shooting at the top of the muscle. Restoring the facial oval causes the forehead to recover.
We also use fractional laser in skin renewal. We are able to stimulate the skin with laser shots focused on the skin at a depth of 1-1.5 microns. We also use the patient's own blood platelets to renew the skin. Thirty years ago, we thought that platelets only help blood clotting, but now we use the growth factor in it for wound healing and skin regeneration. It helps self-repair of the skin. By applying once a month for following three months, it reinforces the skin's self-repair; then, it will be enough for patients to see doctors for checking.
Usually face to face applications are discussed too, but there are many new methods about the body. Can you tell us about these methods?
The focused ultrasound technology, which is also used by many non-operating methods. In this method, the sound waves focus on adipose tissue and deteriorate the fat tissue. Cavitation is quite different from ultrasound. There is an ultrasound head in cavitation; it gives a certain heat energy to the fat cells. This heat energy leads to a number of changes in the wall of the fat cells causing the fat to go away. But cavitation is like putting a margarine in the sun. The oil is melting, but it is still standing there. In the focused ultrasound, the sound wave focusing in the depth of the fatty tissue detonates the fat cell there, eliminating the fat cells. It doesn't damage the healthy tissue around it. Ultrashape HIFU (High Intense Focus Ultrasound) technology is effective in local weight loss, especially in the abdominal area and on the hips.
Do you intend to practice medical aesthetics yourself? What do patients need to pay attention when choosing a doctor?
I try many things on my own. It's not on my face, but I'm trying. I'm using the fractal laser for skin renewal. I don't think I need an extra practice except for it.
There are substances of different quality used in medical aesthetics. The low-quality ones are brought to Turkey in illegal ways or they are produced under the counter. Therefore, these applications should be applied by doctors in medical environments and the patient should choose a reliable physician. The criteria for this are different for everyone, but I don't recommend changing your physician easily.
Do you have patients with extreme demands? How do you direct and guide them?
My suggestion for patients would be; "I try to meet your demands and needs, but I'll be the one who decides." As I stated in the definition of medical aesthetics, the main goal is to achieve a psychophysical balance.
Medical aesthetics is a multi-disciplinary structure. Dermatology, plastic surgery, psychology and psychotherapy and even physical therapy are important. Because a patient with a fat in her abdomen comes in and we say that you should learn how to exercise your muscles by doing Pilates. Medical aestheticist should be their coordinator. By looking at the patient, they should be able to decide on the things they can figure out and get support from other experts.
